Merrill Lynch Wealth Management Questionnaire

This questionnaire represents the first step for Merrill Lynch clients in the long process of securing their financial future.  I designed a user-friendly form with clear levels of hierarchy that make the document easy to navigate, and provided definitions in context where unfamiliar terms appeared. 

Goals Section

In addition to collecting basic data, the goals section of the questionnaire was also intended to help financial advisors gauge who the client is as an investor, their financial literacy and comfort, and the priority of competing goals. We created multiple prototypes, including sortable cards and a drawing exercise prompt, and tested them with users. In the end, financial advisors chose the option with the most open-ended prompt so they could use this tool however they wanted.
